Dr.Phelps,

I've been diagnosed bipolar2. My question is, I was put on topamax 50mg and about a month ago my doc increased it to 100mg within 2 weeks. Now I'm losing weight and having bad panic attacks. My doc wants me to drop 50mg in 4 days and thats makes me nerves. Is that safe?

Dear JoAnn -- 
We don't have all that much experience with Topomax so far, but from what I've seen with it so far, this move is not unreasonable at all.  The max' dose is 400 mg and I've had one patient take quite a bit more than that, so from 50 to zero, on that scale, is not a big step.
